All animals will be seen and treated in accordance with the Veterinary Surgeons Act of 1996 (Section 19) and the Veterinary Surgery Exemptions Order of 2015. This states that treatment provided by a veterinary physiotherapist must be done under referral from a veterinary surgeon. However, musculoskeletal maintenance with the owner's permission for a healthy animal does not require referral from a veterinary surgeon. Although, the animal must still be registered with a veterinary surgeon.

Therefore, we kindly ask that you download the form below and fill in the relevant sections before sending it to your general practice vets (and referral vets if necessary) for completion. Alternatively, we can complete the relevant sections and send them to your vets on your behalf.
